Job Summary The Behavioral Health Supervisor for Healthy Aging and
Wellness Program (HAWP), under the supervision of the PACE and
Senior Service Director will oversee and lead the social services
and case management group. This position plans, organizes and
implements social work services for those participants in the HAWP
program, which include participants enrolled in the Program for
All-inclusive Care of the Elderly (PACE) and Assisted Living
facility. The Behavioral Health Supervisor for HAWP provides
clinical care and functions as a member of the Interdisciplinary
team, pursuant to 42 CFR §§ 460.102(b) and (d), and meets
applicable educational and licensure requirements to represent the
Master's Level Social Worker role. Education - Master’s degree in
Social Work from an accredited school required. Experience - Three
(3) years' experience in behavioral health services delivery
required; administrative experience in areas of supervision
preferred; experience in program development and evaluation
preferred; experience providing behavioral health services in a
PACE setting preferred; experience working with elderly population
preferred. Other Requirement(s) - Active Washington State Licensed
Independent Clinical Social Worker license. Valid WA State Driver’s
license and proof of insurance. Must meet Washington State
requirements and qualifications as an approved clinical
supervisor.
