Job Description:
We are seeking a dedicated and experienced RN Case Manager to join
our team in Port Angeles, WA, for a 13-week contract. The RN Case
Manager will be responsible for coordinating patient care,
developing care plans, and ensuring efficient healthcare
delivery.
Responsibilities:
Conduct comprehensive patient assessments to determine needs and
develop individualized care plans
Collaborate with healthcare providers to ensure coordinated and
holistic care for patients
Monitor patient progress and update care plans as necessary
Educate patients and families on health management and resources
available
Ensure compliance with regulatory requirements and standards of
care
Communicate effectively with patients, families, and the healthcare
team
Utilize case management software to document patient care and
maintain accurate records
Requirements:
Active RN license in the state of Washington
Minimum of 2 years of experience in case management or a related
field
Strong communication and interpersonal skills
Ability to work independently and as part of a team
Experience with electronic health records (EHR) systems
preferred
